About The Position

The Respite Intake/Program Coordinator plays a key role in supporting the Respite Leadership team and Respite programs. Has responsibility for coordinating intake for all respite programs, coordination of Self-Directed budgets and Life Plans, and support to all respite programs as delegated by the Associate Director/Director of Respite. Provides support to the program managers, ensuring administrative expectations are relayed and carried out in each program. Pay for this position: $23.40 / hr - $27.00 / hr The salary of the finalist selected for this role will be set based on a variety of factors, including but not limited to departmental budgets, qualifications, experience, education, licenses, specialty, and training. The above range represents the organization's good faith and reasonable estimate of the range of possible compensation at the time of posting

Requirements

  • At least 5 years’ experience in a position that serves people with intellectual and/or developmental disabilities required.
  • Minimum of High School diploma or high school equivalency; college experience preferred.
  • At least 2 years of Administrative experience.
  • Most demonstrate exemplary verbal and written communication skills as well as organizational skills.
  • Experience in Microsoft Word, Outlook, Excel and Powerpoint required.
  • Ability to maintain composure and remain objective even when dealing with emotional topics, and facilitate crucial conversations when needed. and/or experience with OPWDD regulations.
  • Ability to create and maintain a positive and inclusive work environment built on respect.
  • Valid driver’s license that meets Heritage Christian Services’ motor vehicle record standards, and reliable transportation to and from work.
  • Ability to work direct care if needed.
  • Must be available Monday-Friday until 6:00pm if needed.
  • Flexibility is preferred.
  • Physical ability to lift (over 50lbs) and assist people with any needed personal care; ability to follow through with SCIP-R physical intervention training procedures as needed.

Nice To Haves

  • Experience in Therap and ADP preferred.

Responsibilities

  • Provides clear and timely communication with Respite leadership, family/guardians, care coordinators, fiscal intermediary’s, brokers, OPWDD, HCS clinical supports and other supports as deemed necessary for the intake process.
  • Provide information on respite services to individuals, family members, and care coordinators.
  • You serve as the first and primary person of communication for all respite programs to family, care coordinators and other interested parties.
  • Provide respite application and associated paperwork to interested individuals, family members, and care coordinators.
  • Review initial respite applications and intake paperwork for completeness/thoroughness and to determine if we can safely provide respite services to the individual.
  • Identify additional information that needs to be obtained and follow up with families/MSCs to obtain information.
  • Create/maintain UNR for each individual applying for/on the waiting list for respite services including correspondence regarding the intake process and required documentation.
  • Inform Director of Respite Services when all necessary intake paperwork has been obtained for the individual and forwarding the file for review.
  • Coordinate tours and intake visits at the respite programs as needed.
  • Maintains all communication regarding to intake/Lifeplans/Self-directed budgets to families and care coordinators.
  • Coordinates and communicates with each program manager throughout the intake process to ensure a smooth transition and adherence to all compliance requirements.
  • Annually, to help with all duties related to operation of Camp Heritage including review of applications, selection of campers, obtain all required paperwork from families and care coordinators, complete Advanced First Aid and CPR training and all other Department of Health requirements. Must be available to work camp week.
  • Maintain electronic documentation records for all respite programs, including scanning and uploading hard copies of documentation to Therap when needed.
  • Maintain tracking and compliance of all Self-Directed budgets.
  • Maintain tracking and compliance of all Life Plans.
  • Provide direct support at program sites as needed.
  • Ensure health and safety needs of the respite participants are addressed and met
  • Participate in staff meetings and other treatment or staff related meetings.
  • Serves in agency committees as assigned.
  • Ensure that the Agency Policy and Procedures are carried out appropriately in each program.
  • Ensures compliance with all required trainings.
  • Participates in required trainings/leadership development.
