About The Position
The Resource Associate assists in identifying and utilizing community resources for patients and their families. This role involves liaising with various agencies regarding social work issues and maintaining all pertinent records.
Requirements
- Associate of Arts/Science Degree or equivalent college coursework, in Behavioral and/or Social Sciences, required.
- Demonstrated interest in the field of Social Welfare via educational, previous employment, fieldwork, volunteer work, and/or life experience, required.
Responsibilities
- Assists in identifying and using community resources for patients and/or their families including visits to patient's homes.
- Assists in establishing liaison and communications with other agencies including visits to agencies.
- Maintains a variety of records and reports relating to social work activity
- Performs related duties, as required.
