As a Full-Time Adult Family Home Team lead in Onalaska, WI, you will provide direct support to individuals with disabilities living in Aptiv's Adult Family Home. Four roommates are living in a home under a true roommate arrangement, the feeling is family-like, rather than institutional. Since you would be working with the same number of residents each day, you would get to know them very well. You will also use your strong leadership skills to assist coworkers and help manage the home.

Requirements

  • High school diploma or equivalent
  • Strong verbal and written communication skills
  • Drivers license
  • Flexibility in scheduling must be able to work weekends and holidays.
  • Ability to operate computer equipment; knowledge of various software packages
  • Direct care OR group home experience

Nice To Haves

  • Related two or four-year degree
  • Prior leadership experience

Responsibilities

  • Serve as an advocate and assist individuals in understanding options and communicating their choices regarding living arrangements and work.
  • Act as a trainer and mentor to other staff members.
  • Assist with case management duties and share responsibility for the residents and their success.
  • Collaborate with care teams to ensure the health and safety of the residents.
  • Assist participants in integrating into the community and working environments,
  • Encourage participants in building friendships.
  • Documentation on participant updates, time and billing sheets, goal tracking, medication records, daily logs, etc. and ensure staff are staying up-to-date on documentation as well.
  • Facilitate activity planning in the home, provide transportation, and grocery shop, accompany residents to appointments, and support their goals.
  • Maintain current knowledge and assure compliance with applicable laws, rules, and regulations.
