This position involves providing direct care and support to individuals, responding to their needs during assigned shifts. The role focuses on promoting individual growth across various quality of life areas, including individual control, respect, relationships, community membership, personal growth, accomplishments, and personal well-being. Responsibilities include assisting individuals with their Individual Support Plans, problem-solving, personal and medical care, socialization, leisure activities, interpersonal skills, household management, money management, activities of daily living, and asserting their human and civil rights. The employee will also assist in developing and implementing behavior support plans, know emergency procedures, complete necessary documentation (e.g., Communication Log, Daily Progress notes, Incident Reports, Medication and Treatment Charts), and transport individuals to appointments and community activities. Adherence to Triangle's policies and procedures, maintaining a positive and respectful attitude, effective communication with supervisors, and efficient time management are essential.
