Residential Coordinator (Levels I–III) Are you passionate about supporting individuals with intellectual and developmental disabilities while thriving in a role that blends autonomy, advocacy, and real connection? We’re seeking a Residential Coordinator to oversee and support host home placements, family caregivers, and adults in supported apartment programs. This role requires excellent judgment, compassion, and follow-through. You’ll coordinate services, monitor care quality, respond to client needs, and maintain compliance with a wide range of documentation and regulatory requirements. What the Role Entails Manage a caseload of adults receiving residential services, ensuring their health, safety, and quality of life Oversee care coordination and compliance with HCPF, CDPHE, and other licensing regulations Monitor medical needs and ensure follow-up on protocols, appointments, and PRN medications Collaborate with interdisciplinary teams including case managers, guardians, therapists, and advocates Attend and contribute to clinical meetings, HRC reviews, and other case-related discussions Maintain accurate documentation, perform ongoing file audits, and complete required reporting Conduct unannounced home visits and safety welfare checks in alignment with agency protocol Support the development of person-centered treatment and behavior plans Communicate regularly with clients and providers to build strong, lasting relationships Use professional discretion to solve problems and manage crisis situations in real-time
