What you’ll be doing: You’ll coordinate and oversee all aspects of care for both temporary and permanent residents, ensuring services align with their needs and comply with all applicable Federal, State, and local regulations. This includes developing and implementing individualized nursing care plans, managing medications and treatments, monitoring changes in residents' conditions, and facilitating communication between healthcare providers, residents, families, and interdisciplinary teams. You will also contribute to admission and discharge planning, participate in care conferences and committees, complete documentation such as the Minimum Data Set (MDS), and ensure compliance with regulatory guidelines. In addition, you’ll champion Pacific Retirement Services’ mission, vision, and core values in your daily work, advocate for resident rights, and maintain a focus on achieving each resident’s highest level of functioning. Requirements

  • Registered Nurse (RN) license
  • Strong assessment and care planning skills
  • Ability to communicate with tact, empathy and professionalism

Responsibilities
