About The Position

Performs individual and group social service functions for VOAMN Highrise Social Service Program as assigned in the Minneapolis Public Housing (MPHA) highrises and family housing. Essentials: Provide advocacy, information and referral and case management to MPHA residents to enable them to live as independently as possible. Together with supervisor, evaluate and adjust services in the buildings as needed to ensure best outcomes for clients and residents. Outreach to individuals and groups within assigned buildings to ensure that residents and potential clients are aware of available services. Interpret and promote other VOA services and community programs to residents so that they are knowledgeable about resources available to them. Provide group services to residents to meet identified needs. Maintain case file documentation to ensure that professional standards of documentation are followed, and maintain client medical information, financial records and personal information in a confidential manner. Collaborate with other VOA services and community professionals to ensure that a wide network of resources is available for Highrise clients. Participate in case review sessions on a regular basis to help guide and strengthen social service skills. Participate in training opportunities to maintain and improve professional social service skills. Assures client privacy is maintained by being knowledgeable and following HIPAA privacy and security regulations. Work as many hours as needed to complete the job. Use internal electronic health record and/or other electronic documentation systems to complete documentation as required Ability to respectfully interact with a wide variety of people Analytical and problem-solving skills Local travel required Sit, stand, walk, lift 25 pounds, do concentrated computer work

Requirements

  • Bachelors degree in Human Services or related
  • 2-3 years of experience working in general social services or case management
  • Analytical and problem-solving skills
  • Local travel required
  • Sit, stand, walk, lift 25 pounds, do concentrated computer work
  • Work as many hours as needed to complete the job

Nice To Haves

  • Experience working with elderly or disabled clients
  • Experience providing social services to diverse populations
