About The Position

This role focuses on providing pharmacy related services in a hospital or retail setting. In addition, this role focuses on performing the following Pharmacy Services duties: Delivers pharmaceuticals. Includes professionals who are trained and sometimes licensed to dispense medicine/controlled substances. A professional individual contributor role that may direct the work of other lower level professionals or manage processes and programs. The majority of time is spent overseeing the design, implementation or delivery of processes, programs and policies using specialized knowledge and skills typically acquired through advanced education. An entry level role that applies broad theoretical job knowledge typically obtained through advanced education. May require the following proficiency: work is closely supervised, problems faced are not typically difficult or complex, and explains facts, policies and practices related to job area. This one-year post-graduate training program accredited by the American Society of Health System Pharmacists (ASHP) builds on Doctor of Pharmacy (PharmD) education to develop clinical pharmacists who are responsible for medication-related care of patients with a wide range of conditions, eligible for board certification and eligible for postgraduate year two (PGY2) pharmacy residency training. Residents are prepared as pharmacists for a clinical practice that integrates direct patient care, drug distribution, teaching and management. The residency program’s purpose is achieved by utilizing the four competency areas and their associated educational goals and objectives as required by the ASHP Standard. The competency areas are: (1) patient care; (2) advancing practice and improving patient care; (3) leadership and management; and (4) teaching, education, and dissemination of knowledge

Requirements

PharmD from an ACPE accredited program.
Unrestricted Pharmacist license within the first 90 days of employment.
Ability to work independently with minimal supervision and as part of a team.
Superior verbal and written presentation skills.
Superior communication skills, including developing and giving presentations, both 1-1, and group.
Knowledge of integrated health networks and health plan drug formularies.
Willingness and ability to learn and utilize new technology and procedures that will continue to develop in their role and throughout the organization.
Project and resource management skills.
Creative thinking and problem-solving skills.
Ability to understand importance of and respect for the confidentiality of all patient information in accordance with applicable standards and regulations.
Knowledge of clinical best practices and standards for optimizing medication use.
Proficiency in Microsoft Office applications, including Access, Excel, PowerPoint, and Word.
Ability to work with all ages, backgrounds, ethnicities, and life experiences.
A robust understanding of chronic health condition management and population management.
Motivational Interviewing proficiency.
Ability to prioritize and resolve critical issues efficiently and effectively.
Detail oriented with strong organizational skills and multi-tasking abilities.
Very strong working knowledge and proficiency with technology and business software (Microsoft Office).

Nice To Haves

Experience in drug utilization review and analysis.
Experience with Electronic Medical Records.

Responsibilities

Upon completion of an orientation period, the resident rotates though various areas of the Department of Pharmacy (Medicine, Pediatrics, Critical Care, Cancer Care, Central Operations).
Reviews and approves medication orders for all Medical Center patients. This includes weekend, holiday, evening coverage as described in departmental policies and procedures.
Provides continuing education for professional staff (e.g., pharmacists, physicians, nurses).
Educates other health care providers by publishing drug information articles in the pharmacy newsletter.
Participates in periodic reviews of therapeutic classes of drugs and present findings to the Pharmacy and Therapeutics Committee.
Prepares and presents to the appropriate committee’s evidence-based reviews of drugs requested for formulary addition.
Initiates and facilitates completion of drug use evaluations.
Detects and reports medication errors and adverse drug reactions in a timely manner.
Provides accurate, adequate and timely drug information to professional staff.
Conducts medication history interviews, reconcile patients’ medication therapies and provide drug information to patients and their families.
Coordinates care to ensure safe and effective transitions from inpatient to outpatient care.
Documents in the medical record patient evaluations and recommendations, medication reconciliation and medication histories.
Documents pharmacist interventions according to established policies.
Participates a number of longitudinal activities including, a teaching seminar and student teaching (e.g., case presentations, journal clubs, problem-based learning), and completion of a year-long residency project.
