About The Position

The Resident Care Coordinator is responsible for coordinating client care related to scheduling of and communication following appointments, transportation, and insurance. In addition, this position guides and assists individuals to live and function as independently as possible in the community and participates in the development and implementation of individual program plans and coordination for the provision of specialized services. This position requires the position to travel between sites on a regular basis. This position requires at least an associates degree with 4 years of work experience, OR a bachelor's degree with 2 years of experience, OR a masters degree and 1 year of work experience. Schedule: Mon - Fri 8am - 4pm, depending on Lab work and other scheduled doctor appointments.

Requirements

  • A master’s degree or above from an accredited college or university and 1 year of work experience working directly with individuals with an intellectual disability or autism.
  • A bachelor’s degree from an accredited college or university and 2 years of work experience working directly with individuals with an intellectual disability or autism.
  • An associate’s degree or 60 credit hours from an accredited college or university and 4 years of work experience working directly with individuals with an intellectual disability or autism.
  • Quickmar software
  • Complete criminal background
  • Name checked in the registries. (OIG exclusions database, Child Abuse Registry, and Offenders Against Individuals with Developmental Disabilities)
  • Drug testing
  • Education verification and other credentialing based on position requirements.
  • Proof of employment history or references (if required)
  • Proof of driver's license, driver's insurance, and vehicle, IF required for providing transportation for individuals.

Responsibilities

  • Coordinate and schedule medical, psychiatric and dental/vision appointments with doctors' offices, health care providers, laboratories along with communicating and liaison with physicians, nurses and other relevant behavioral health and health care professionals regarding the medical needs of the resident. ( 65%)
  • Track all physical, health care ,dental , vision appointment . (10%)
  • Prepare and complete all necessary documentation in accordance with program standards within required time frames as required. ( ie. MA 51) (10%)
  • Perform Direct Support Services when needed (10%).
  • Work on computer to perform the above duties - ongoing
  • Other duties may be assigned as necessary due to the needs of the program and residents served.
