Provides expertise in the areas of release of information and data integrity to post-acute care facilities within the Sanford footprint including nursing facilities, home health agencies, and assisted living facilities. Serves as a subject matter expert to post-acute care administrative leaders and HIM directors/coordinators. Provides management and support for complex medical records involving legal/attorney requests, court orders, subpoenas, and other high-risk requests. Identifies high-risk requests and responds appropriately by engaging with appropriate parties. Manages and processes complex requests for medical record information including consulting with Legal, Compliance, Risk, Clinical, and other teams as appropriate. Within the post-acute care setting, processes and responds to requests involving electronic medical record (EMR) information from EMR systems and archived paper medical records. Provides direction to administrators, HIM directors/coordinators in the fields regarding ROI practices, particularly on complex release of information issues. Interacts with Administrators and department leaders. For post-acute settings, directs and guides management and correction of EMR documentation errors in accordance with established policies and procedures until corrections are complete. Provides data integrity expertise to the Master Patient Index (MPI) including duplicate monitoring and mitigation strategies surrounding patient safety and privacy. Develops and implements error prevention strategies. Manages and oversees the medical record archive. For post acute-care settings, serves as a subject matter expert to the field. Interacts with Administrators, HIM directors/coordinators, and Corporate Services to address complex requests and scenarios. Manages and responds to complex requests for medical records in an efficient manner within established timeline standards, maintaining quality and accuracy. Safeguard and protect the patient's right to privacy. Maintain strict confidentiality of patient information in compliance with HIPAA and state and federal regulations. Provides direction and guidance in accordance with policies, procedures, and regulations. Maintain in-depth knowledge of all applicable state and federal laws, HIPAA privacy and security regulations, as well as enterprise policies and procedures pertaining to the release of protected health information in post-acute care settings. Ensure all disclosures are documented. Interpret and adhere to legal requirements and established procedures that govern release of information. Critically analyze, interpret and apply laws and regulations to determine whether requests and authorizations are in compliance. Maintain understanding of the definition and components of the legal medical record. Utilize a variety of software and applications in order to fulfill requests. Understands the complexity of medical records and location of various documents for post-acute care settings. Builds and maintains positive relationships and works collaboratively with Administrators, HIM directors and stakeholders to analyze problems and propose recommended solutions that meet both business and technical needs. Must be able to work with all clinical teams to resolve data integrity issues including MPI and documentation errors. Maintains required information for tracking through follow up and resolution. Prepares, presents and discusses reports and studies with management, other teams and individuals. Understand healthcare terminology, business processes, and user issues associated with post-acute care practices. Demonstrates analytical skills to help identify and communicate possible prevention strategies based on data analysis. Uses critical thinking and problem solving skills to identify and determine appropriate course for correction process. Directs amendments to post-acute care medical records when a patient amendment request is approved through the Privacy Office. Validates the information at conclusion of the correction assuring that integrity of record is intact. Tests new functionality related to post-acute care EMR documentation systems. Provides training through a variety of formats (webinars, in-person, etc.). In the context of post-acute care delivery, must have knowledge of medical record content and management and good working knowledge of the EHR either through experience or education. Must have a working knowledge of PC systems, including knowledge of MS Office products. Demonstrated ability to communicate and work effectively with internal and external customers showing poise, tact and professionalism at all times.
