SCHEDULE: Full-Timer/ 40 Hours Monday- Friday 8:00am- 5:00pm Ranked in the top 25 percent of all home care agencies in the United States, Banner Home Care is the largest nonprofit, free-standing home care agency in Arizona. POSITION SUMMARY This position is accountable for the quality of rehabilitative services delivered by self and through the supervision of team members who are under their direction. This position utilizes specialized knowledge, judgment, and rehabilitation skills necessary to assess data and plan, provide and evaluate care. Manages personnel to ensure appropriate allocation of resources in providing quality patient care and appropriate rehabilitation care of patients in accordance with organizational policies, regulatory bodies, and leadership direction.

Requirements

  • Graduate of an accredited program in Occupational, Physical, Audiology or Speech Therapy required.
  • Must hold a current license or certification in respective discipline.
  • A minimum 2 years of Home Health experience required.

Nice To Haves

  • Additional management or supervision experience preferred.
  • Additional experience in Home Health a plus.
  • Additional related education and/or experience preferred.

Responsibilities

  • Provides, facilitates and supervises patient centered care and care coordination to ensure patient care needs are met while maintaining quality standards and regulatory compliance in a financially responsible manner.
  • Serves as a resource to patients, families, physicians, and staff by providing hands on care, clinical support, managerial support, interpreting policies, and facilitating care.
  • Hires, leads, mentors, develops, and evaluates staff in a manner that promotes engagement, productivity, and ensures competencies.
  • Supervises and delivers care based on the best evidence available and may participate in research activities within clinical practice.
  • Ensures effective use of resources, equipment and supplies.
  • Leads, promotes, and participates in interdisciplinary patient care planning and education.
  • Formulates, implements, and supervises creation of a plan of care, including the discharge plan, through direct patient care, care coordination, delegation ad supervision utilizing assessment data and patient, family and health team input.
  • Ensures documentation, assessments, and care plans are timely and in accordance with policy.
