Rehabilitation Services is a multidiscipline team comprised of Cardiac Rehab (CR), Occupational Therapy (OT), Physical Therapy (PT), and Speech Therapy (ST) with collaboration and assistance from the Patient Access Department. The role of the Rehabilitation Coordinator involves working closely with the leaders of these departments to deliver top-tier community services. This position entails supervising daily clinic operations, coordinating activities among office and therapy staff, creating work schedules, and maintaining an optimal workflow. The Rehabilitation Coordinator is responsible for various crucial tasks, including participating in annual reviews, assistance with hiring processes, assisting and scheduling performance improvement plans including community partnerships and maintaining positive relationships with referral sources. Makes recommendations for policies and procedures and quality initiatives for the rehab departments. Effective communication is vital, as the coordinator keeps all rehabilitation staff informed about progress and potential challenges. Efficiency and time management is another key aspect, involving the oversight and evaluation of patient throughput for multiple disciplines, setting, and locations. Furthermore, the coordinator role extends to the preparation of essential reports for the hospital, including Finance, Quality Assessment and Performance Improvement (QAPI), and Human Resources. These measures align with both regulatory and hospital guidelines. Maintaining seamless communication across various rehab service lines and locations is paramount, to maintain cohesion and effectiveness of the entire rehabilitation department.
