About The Position

The registered nurse plans and coordinates home care services and is experienced in nursing, with emphasis on community health education/experience. The registered nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.

Requirements

  • Must have graduated from an approved school of nursing and be licensed in the State of Wisconsin.
  • Minimum of two (2) years of experience, at least one of which is in public health or home care nursing is preferred.
  • Must be a licensed driver with an automobile in good working order and insured in accordance with state or organization requirements.
  • Must be self-directed and able to work with minimal supervision.
  • Must demonstrate excellent observation, problem solving, verbal/written communication skills and nursing skills, per competency checklist.
  • Must demonstrate ability to organize and prioritize workload independently.
  • Responsible for supervising home health aides.
  • Ability to endure prolonged or considerable walking/standing and lift/position/transfer patients, supplies and equipment.

Responsibilities

  • Completes an initial assessment of patient and family to determine home care needs.
  • Following a complete assessment, develops a written plan of care for each patient assigned to caseload.
  • Initiates the plan of care and assumes responsibility for the ongoing interdisciplinary assessment and expansion of the individualized plan of care in partnership with the patient/representative/caregiver.
  • Assesses patient’s condition, re-evaluates and updates plan of care as necessary.
  • Initiates proper preventive and rehabilitative nursing procedures and provides services as ordered by the physician and indicated in the plan of care.
  • Provides skilled nursing care.
  • Counsels the patient and family in meeting nursing and related needs.
  • Provides patient and caregiver education as appropriate per assessment and plan of care.
  • Prepares clinical notes and completes at least 90% of all charting while in patient’s home.
  • Communicates with the physician who is responsible for the home health plan of care and other health care practitioners related to the current home health plan of care.
  • Obtains all necessary physician orders and collaborates with physician as patient’s needs dictate.
  • Attends case conferences to discuss multidisciplinary team responsibilities, patient progress, plans for continued care, etc.
  • Re-evaluates the need for continued care on an ongoing basis and initiates recertification as necessary.
  • Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient.
