Responsibilities may include:

Requirements

  • Active RN license in DC and Maryland
  • Minimum 1 year of nursing experience; LPN‑to‑RN graduates welcome
  • Strong communication skills and professional presentation
  • Reliable transportation (mileage reimbursed)
  • Comfortable with digital charting (iPad/computer)
  • Willingness to travel throughout assigned service area

Responsibilities

  • Patient Care & Advocacy Complete start‑of‑care assessments and patient onboarding as assigned
  • Develop or update individualized care plans based on medical needs and patient/family preferences
  • Monitor patient progress and communicate status updates to physicians and care teams
  • Educate or supervise family members assisting with care to ensure safety and confidence in the home
  • Team Guidance & Support Provide instruction to Home Care Aides, Personal Care staff, and nurses regarding care plans and best practices
  • Conduct PRN supervisory visits to ensure quality care and compliance
  • Offer feedback on documentation and care delivery as needed
  • Participate in staff orientation or training sessions when scheduled
  • Compliance & Quality Assurance Ensure adherence to OSHA, DMAS, CCC Plus, and state regulations
  • Complete accurate and timely clinical documentation
  • Participate PRN in interdisciplinary meetings or quality initiatives
  • Community Engagement (As Needed) Represent Care Advantage positively with referral partners and community programs
  • Support integration efforts during organizational growth when requested
