A Day in the Life Manages an assigned caseload from admission through discharge using a framework of assessment, planning, coordination, and evaluation. Responsibilities include completing comprehensive assessments (including OASIS when required), developing and updating individualized plans of care, and providing skilled nursing services across preventative, rehabilitative, palliative, and end-of-life needs. Educates and counsels patients and families on disease processes, symptom management, and self-care, and coordinates care with interdisciplinary team members such as Home Health Aides, Therapies, Social Work, Chaplaincy, and Bereavement Services. This position also supervises LPNs and paraprofessionals, communicates changes in patient condition to physicians and team members, secures necessary equipment and supplies, and maintains thorough, timely documentation within the electronic medical record.
