This position provides direct patient care to patients using the nursing process in accordance with applicable scope and standards of practice. Requirements

  • Associate’s degree Required
  • Graduate of a Credited Nursing Program
  • Current RN to practice in the state of Texas
  • American Heart Association BCLS (New employees with BLS certification from another organization have 30 days to complete American Heart Association version)
  • Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision
  • Must be able to work in a stressful environment and take appropriate action
  • Rehab experience required

Responsibilities

  • Performs plan of care intervention, including medication administration, specimen collection, clinical treatments, as well as other medical care treatment.
  • Documents patient care given.
  • Monitors, records, and communicates patient condition as appropriate to care team, physician, patient, and family.
  • Assists patients with performing activities of daily living, including personal hygiene, elimination, nutrition, and ambulation.
  • Collaborates as needed across disciplines to coordinate patient care, including patient transfer, discharge, referral and spiritual/psychosocial support needs.
  • Evaluates learning needs of patient and/or family and provides patient/family education appropriate to age, culture, condition, and circumstances.
  • Works as an advocate for the physical and emotional well-being of the patient.
  • Assesses, plans, implements, and evaluates care for a given population based upon age specific and cultural needs in a collaborative multidisciplinary approach embracing quality and safety.
  • Assures quality of care by measuring health outcomes against patient care goals and standards; making or recommending necessary adjustments; following hospital philosophies and standards of care set by Texas State Board of Nursing, and other governing agency regulations.
  • Coordinates the delivery of patient care for the Rehab Unit, facilitates problem solving with staff members and communicates necessary issues to nursing administration and the Program Manager.
