About The Position

This position at Olmsted Medical Center is for either a Registered Nurse (RN) or a Licensed Practical Nurse (LPN) in an outpatient General Surgery/Urology setting. The role is 0.9 FTE on a day shift. Both LPNs and RNs are responsible for patient assessment, participating in the development and implementation of care plans, ensuring a safe and therapeutic environment, advocating for patients, collaborating with other healthcare providers, and managing patient information in compliance with regulations. LPNs conduct focused assessments and perform delegated tasks, reporting changes to an RN or licensed healthcare provider. RNs provide comprehensive assessments, develop and coordinate integrated care plans, implement independent nursing actions, and manage/supervise nursing practice. Requirements

Graduate from an accredited school of practical nursing (for LPN)
Current Minnesota LPN license required (for LPN)
BLS certification
Completion of an accredited professional nursing program (for RN)
Current Minnesota Registered Nurse license (for RN)
ACLS certification (Northwest Clinic and Hospital locations only including the departments of Internal Medicine, Family Medicine, Acute Care, Cardiology, Pain Management, and Pediatrics) (for RN)

Nice To Haves

Previous clinic experience preferred (for LPN)
Minimum of one year nursing experience preferred (for RN)

Responsibilities

Conducts a focused assessment of the health status of an individual patient through the collection and comparison of data to normal findings and the individual patient’s current health status.
Reports changes in health status and responses to interventions in an ongoing manner to a registered nurse or the appropriate licensed healthcare provider for delegated or assigned tasks.
Participates with other healthcare providers in the development and modification of a plan of care.
Determines and implements appropriate interventions within a nursing plan of care or when delegated or assigned by a registered nurse.
Implements interventions that are delegated, ordered, or prescribed by a licensed healthcare provider.
Assigns nursing activities or tasks to other licensed practical nurses.
Assigns and monitors nursing tasks or activities to unlicensed assistive personnel.
Provides safe and effective nursing care delivery.
Promotes a safe and therapeutic environment.
Advocates for the best interest of the patient.
Assists in the evaluation of responses to interventions.
Collaborates and communicates with other healthcare providers.
Provides healthcare information to individual patients.
Provides input into the development of policies and procedures.
Accountable for the quality of care delivered, recognizing the limits of knowledge and experience; addressing situations beyond the nurse’s competency; and performing to the level of education, knowledge, and skill ordinarily expected of an individual who has completed an approved practical nursing education program.
Maintains OMC Organization Competencies and Nursing Competencies pertinent to area of practice.
Manages information in accordance with state and federal regulations.
Communicates effectively.
Provides a comprehensive assessment of the health status of a patient through the collection, analysis, and synthesis of data used to establish a health statue baseline and plan of care. The RN also addresses changes in the patient condition.
Collaborates with the healthcare team to develop and coordinate an integrated plan of care.
Develops nursing interventions to be integrated into the plan of care.
Implements nursing care through the execution of independent nursing actions.
Delegates nursing tasks or assigns nursing activities to implement the plan of care.
Evaluates responses to interventions and the effectiveness of the plan of care.
Collaborates and coordinates with other healthcare professionals in the management and implementation of care within and across care settings and communities.
Provides health promotion, disease prevention, care coordination, and case finding.
Designs and implements teaching plans based on patient need and evaluates teaching effectiveness.
Participates in the development of healthcare policies, procedures, and systems.
