About The Position

The Hospital for Behavioral Medicine (HBM) in Worcester, MA, is a cutting-edge 120-bed inpatient facility and a leading provider of exceptional therapeutic care, prioritizing patient comfort and safety. In proud collaboration with UMass Memorial Health Care, HBM also offers an outpatient partial hospitalization program. The Registered Nurse (RN) Intake is a registered professional nurse responsible for assessing and evaluating patients being admitted to the hospital, taking physician orders, and completing medication reconciliation. This role requires good clinical skills and the ability to work independently with minimum supervision, serving children, adolescents, adults, and senior populations. HBM is committed to accessible care, emphasizing availability irrespective of one's ability to pay.

Requirements

  • Graduate from an accredited RN program.
  • Currently licensed to practice by the Massachusetts State Board of Nursing.
  • Must pass a medication administration exam before administering medications.
  • CPR certification within 30 days of employment.
  • Successful completion of Management of Aggressive Behavior including restraint and seclusion policies, within 30 days of employment, and prior to assisting in restraining procedures.
  • Must have a working knowledge of the DSM-X, the Mental Status Exam, and the Addiction and Recovery field.

Nice To Haves

  • Bachelor's Degree preferred.
  • A minimum of three (3) years experience in a psychiatric facility preferred.

Responsibilities

  • Assess and evaluate patients being admitted to the hospital.
  • Take physician orders.
  • Complete medication reconciliation with the physician in accordance with the nursing process and the standards of care and practices.
  • Complete the Nursing Assessment documenting patient information, medications, medical history, vital signs, and skin assessment.
  • Work with children, adolescents, adults, and senior populations.
  • Be familiar with common diagnoses with these population groups and familiar with specific assessment and interviewing skills for each population group.
  • Call physician for admission and medication orders.
  • Complete Medication Reconciliation by patient/family/pharmacy verification.
  • Report any signs or symptoms of infection, illness, medical instability to physician after assessment.
  • Document patient information and nursing care in the provision of nursing services.
  • Document and reassess any significant changes in the patient's condition in compliance with facility policy.
  • Walk patient to unit and give report to the unit nurse.
