Position Summary: The Case Manager is responsible and accountable for the management of care for an assigned patient population by service line and across the continuum of care. The Case Manager, works to achieve daily clinical, quality, and cost outcomes by providing well-coordinated experiences for patients/families through the synchronization of care activities of multiple disciplines and negotiation with third-party payers. Serves as a liaison between the patients to community resources, providers, and supports a smooth transition from emergency department to home or continued care facility, with a goal of patient satisfaction, continuity of care across the continuum, avoiding unnecessary admissions, and reducing and/or eliminating readmissions. Performs services according to the policies, procedures, philosophy, and objectives of the department and the hospital. Works cooperatively within the hospital emergency department to create a system of quality healthcare. The ED Case Manager has critical thinking skills and can plan, implement, and evaluate individual patient care programs. The ED Case Manager has strong interdisciplinary communication skills and can provide emotional stability to cope with human suffering, emergencies, and other stressors. Demonstrates use of clinical skills and expertise in emergency department and/or acute care nursing, discharge planning, and post-hospital follow-up care with providers and home services. Serves as a liaison and patient advocate in navigating the healthcare system. Provides patient education and support utilizing knowledge of adult learning competencies and education resources to support complete, connected, and comprehensive care. Communicates effectively with diverse audiences including professional staff, patients, community resources, and other services as needed. Possesses knowledge of supportive care principles and managed care. Ability to prioritize patient needs and offer guidance to achieve optimal continuity of care.
