We are seeking a dedicated and experienced RN Case Manager to support one of our top healthcare clients in Tallahassee, FL. This is a full-time opportunity offering competitive pay , stackable shift differentials , and a generous $10,000 sign-on bonus . Position Summary As a Registered Nurse Case Manager , you will be at the heart of the interdisciplinary care teamensuring patients receive optimal, efficient, and high-quality care from admission through discharge. This role blends clinical judgment, advocacy, and coordination to improve patient outcomes, support utilization review, and drive seamless transitions of care. ️

Requirements

  • Active RN license in Florida (or compact license with FL endorsement within 90 days)
  • Associate Degree in Nursing required
  • Minimum of 3 years of recent Case Management experience in an acute care setting OR 3 years of experience in Med/Surg, Telemetry, Neuro, ICU, PCU, or ED
  • Candidates with Home Health or Insurance CM experience must have at least 1 year of acute care experience within the last 5 years

Nice To Haves

  • BSN preferred
  • Certification in Case Management, Utilization Review, or Nursing preferred

Responsibilities

  • Conduct comprehensive assessments of patients medical and psychosocial needs
  • Develop, implement, and monitor individualized care management plans
  • Collaborate with the care team to ensure appropriate level of care, resource utilization, and discharge planning
  • Facilitate interdisciplinary team meetings to align care strategies
  • Evaluate medical necessity and level of care using evidence-based criteria
  • Act as liaison between patients, families, providers, and payers
  • Identify potential barriers to care and proactively work to resolve them
  • Maintain compliance with all regulatory, quality, and documentation standards
  • Provide education and support to patients and families, especially for those with chronic or complex conditions
  • Coordinate referrals to community resources, disease management programs, and support services
  • Track trends, analyze data, and make process improvement recommendations
