The Quality Assurance Coordinator is responsible for assisting in overall quality improvement activities for Nascentia Health Options and any of its Subsidiaries. Performance Responsibilities and Standards: Quality Improvement 1. Reviews clinical records. a. Participates in review of chart audits utilizing relevant databases and reports (if applicable) and best practice following policies and procedures b. Reviews client records, orders and other communication for quality and safety of care. c. Analyzes documentation and reports trends to Supervisor. 2. Follows up with staff/management regarding issues identified in review process. a. Assist clinical leadership in working with clinical staff on questions, Uniform Assessment System (UAS) education/clarification, documentation issues, and clinical care interventions. b. Communicates with Supervisor regarding issues or concerns with clients and/or staff. c. Follows up with staff regarding documentation deficiencies and/or discrepancies with UAS and other documentation sources. d. Participates in formal and informal training opportunities for staff. 3. Performs quality improvement activities as assigned. a. Communicates agency policies to staff as needed b. Promotes quality improvement as it relates to the activities of staff in accordance with Nascentias policies and procedures. c. Performs focused audits, based on agency need and as directed by Supervisor in support of positive organizational, MLTC Plan outcomes and regulatory requirements. 4. Maintains knowledge of and adherence to agency policies, procedures, along with quality improvement standards. a. Demonstrates knowledge and use of agency policies and procedures. b. Demonstrates knowledge of quality improvement standards. c. Demonstrates knowledge of Universal Assessment Systems. d. Has a working knowledge of Medicare/Medicaid and private insurance 5. Other expectations. a. Provides excellent customer service. b. Maintains client confidentiality. c. Maintains positive relationships with co-workers, administrative staff, clinical staff, families and other entities. d. Keeps Supervisor informed of problems as they arise. e. Attends meetings and conferences as needed. f. Perform all other duties as assigned.
