About The Position

Home Health Registered Nurse (RN) – Rockford & Surrounding Areas
Abcor Home Health is a family-owned agency looking for a compassionate Home Health Registered Nurse to join our team. If you enjoy flexible scheduling, one-on-one patient care, and working in a supportive environment, we’d love to hear from you.
Position Details
Territory: Rockford & nearby areas (within ~30 minutes preferred)
In-home visits (30–45 minutes each)
Minimum 5 visits per week
Training provided (approximately 2 weeks in Arlington Heights, based on experience)

Requirements

• RN license (Associate’s Degree in Nursing)
• Home Health experience required (OASIS preferred)
• Valid Driver’s License
• Able to work independently
• Strong communication and patient-care skills

Responsibilities

• Perform in-home patient assessments
• Create and follow individualized care plans
• Provide skilled nursing care (wound care, IV/PICC, G-tube, Foley)
• Educate patients and caregivers
• Complete OASIS documentation
• Communicate with physicians and care teams
