About The Position
Abcor Home Health is hiring a compassionate Home Health LPN to provide one-on-one care in patients’ homes. Enjoy flexible scheduling, supportive leadership, and competitive pay. Details Territory: Rockford & nearby areas (within ~30 minutes preferred) In-home visits (30–45 minutes) Minimum 5 visits/week Training in Arlington Heights (~2 weeks, based on experience)
Requirements
- Active LPN license
- Home Health experience (OASIS preferred)
- Valid Driver’s License
- Strong communication and patient-care skills
Responsibilities
- Perform in-home patient assessments
- Follow individualized care plans
- Provide skilled care (wound care, G-tube, Foley; IV/PICC if within scope)
- Educate patients and caregivers
- OASIS documentation
- Team communication
