About The Position

POSITION SUMMARY The MMST Registered Nurse (RN) provides comprehensive nursing and psychiatric support to assigned clients experiencing homelessness by participating in treatment planning meetings, monitoring and documenting client responses, and maintaining accurate medical records in compliance with regulatory standards. This role includes delivering group education sessions, conducting medical histories, coordinating incident reporting. The MMST RN also responds to medical and psychiatric emergencies, communicates client status to physicians and care teams, provides in-service training to staff, and ensures all documentation and clinical activities meet applicable county and state regulatory requirements. DUTIES, RESPONSIBILITIES, AND ESSENTIAL FUNCTIONS Participates in meetings and conferences regarding diagnosis and treatment plans for assigned caseload.Obtains medical history of clients and assists in administering specialized therapeutic treatments as ordered by the physician. Maintains documentation of client’s clinic visits and medical progress. Notifies the physician and relatives in case of an emergency. Coordinates incident reporting procedures and ensures proper documentation of incident reports according to Suffolk County DOH and NYS OMH regulations.Delivers group therapy sessions using educational tools (e.g., films and visual aids) to promote client understanding of topics such as hygiene, personal grooming, family planning, and diet/weight management. Coordinates with other programs and departments to ensure quality care is delivered to the client. Completes assessments for clients to determine appropriate level of care and if any immediate needs must be addressed. Assists individuals with accessing community resources to ensure needs are met.Provides emergency medical and psychiatric care to clients as needed. Provides medical in-service programs to staff members which may include but are not limited to first aid, pharmacology, and discussions of specific diseases.Reports any non-medical issues to the Clinic Director and reports any medical or psychiatric issues to the Medical Director and/or Clinical Psychiatrist.Ensures that all required documentation (paper and electronic) is completed /processed appropriately, in a timely manner, and meets quality and compliance thresholds. Performs other duties as assigned. Requirements

  • Associate’s degree in nursing required; bachelor’s degree in nursing preferred.
  • Experience with the mental health and homeless population preferred.
  • Valid NYS Registered Nurse license
  • Must have an ability to establish positive working relationships with colleagues, management, clinicians, external providers and all other valued stakeholders
  • Proficient computer skills with the ability to learn and use various applications
  • Demonstrated ability to work in a team environment with flexibility, keeping others informed and anticipating needs is essential
  • Must be efficient, well organized, and have excellent communication skills
  • Valid Driver’s License and ability to satisfy criteria to be an agency driver required
  • Employee is required to sit, walk and stand for prolonged period of time (at least 8 hours).
  • Employee is required to effectively communicate both verbally and in writing.
  • Employee is required to operate standard office equipment such as computers, phones, photocopiers, fax and filing cabinets.
  • Employee is required to reach with hands and arms.
  • Employee is required to bend, stoop, kneel, crouch or stand on a stool as needed.
  • Employee is required to be able to lift 50 pounds at a time.
