About The Position

The Registered Nurse (RN) – Long Term Care is responsible for delivering high-quality nursing care to residents in a skilled nursing environment. This role includes clinical care, care planning, documentation, and collaboration with interdisciplinary teams to ensure optimal patient outcomes.

Requirements

Active Registered Nurse (RN) License
Knowledge of geriatric/long-term care nursing practices
Strong communication and interpersonal skills
Ability to assess, analyze, and respond to patient care needs

Nice To Haves

Open to recent graduates or candidates with less than 1 year of experience
Experience in Skilled Nursing Facility (SNF) or Long-Term Care (LTC) settings

Responsibilities

Administer medications and treatments as prescribed within scope of practice
Perform initial nursing assessments and assist with MDS and care plan development
Monitor patient conditions and respond to changes appropriately
Assist residents with Activities of Daily Living (ADLs) as needed
Maintain accurate clinical documentation of assessments, treatments, and patient progress
Participate in shift reports and ensure effective communication of patient information
Follow all federal, state, and facility regulations and nursing standards
Communicate effectively with residents, families, and interdisciplinary care teams
Provide guidance and limited supervision to support staff
Participate in staff training and in-service programs
Utilize continuous improvement methodologies for workflow efficiency
Apply problem-solving techniques to enhance patient care delivery
