About The Position

The Licensed Practical Nurse (LPN) – Long Term Care is responsible for providing direct patient care, administering treatments, and supporting care planning for residents in a skilled nursing environment. This role ensures quality care delivery through clinical expertise, documentation, and collaboration with interdisciplinary teams.

Responsibilities

  • Administer medications and treatments as prescribed within scope of practice Includes oral, enteral, parenteral, and other medication forms
  • Perform treatments such as wound care, tracheostomy care, gastrostomy/jejunostomy care, and catheter care
  • Monitor and report effectiveness of medications and treatments
  • Assist residents with care needs and respond to changes in condition
  • Ensure adequate supply of medications and clinical materials
  • Perform admission assessments for new residents
  • Assist with MDS assessments, quarterly reviews, and care plan development
  • Maintain accurate clinical and administrative documentation
  • Participate in shift reports and handoffs
  • Communicate effectively with residents, families, and interdisciplinary teams
  • Share critical updates regarding patient conditions and care needs
  • Participate in Quality Assurance and Performance Improvement (QAPI) programs
  • Adhere to all federal, state, and facility regulations
  • Follow best practices in medication management and patient safety
