About The Position

"Navitas Healthcare, LLC" is seeking a compassionate and skilled Licensed Practical Nurse (LPN) to provide high-quality care in a Skilled Nursing Facility (SNF) or Long-Term Care (LTC) setting. The LPN will work under the direction of the RN or Nursing Supervisor to ensure resident safety, comfort, and adherence to individualized care plans.

Requirements

• Current LPN license in the state of practice.
• Basic Life Support (BLS) certification.
• Valid driver’s license.
• Covid-19 vaccination card.
• Updated professional resume.
• Completion of an accredited Licensed Practical Nurse program.

• Minimum 1 year of experience in a SNF, LTC, or similar clinical setting preferred.
• Familiarity with electronic health record (EHR) systems.
• Strong understanding of nursing procedures and safety protocols.

Responsibilities

• Administer medications and treatments as prescribed.
• Monitor, record, and report changes in residents’ conditions.
• Assist with daily living activities, including hygiene, mobility, and nutrition.
• Collaborate with interdisciplinary teams to develop and implement care plans.
• Maintain accurate and timely documentation in resident medical records.
• Respond to emergencies and provide first-line interventions as needed.
• Educate residents and families on care routines and health management.
