About The Position

Our teams work on cutting-edge programs like hospital-at-home, mobile integrated healthcare, post-discharge stabilization, and rapid response for urgent-but-not-emergent situations. As a Registered Nurse on our Community-Based Care Team, you’ll practice high-level nursing in an environment that values your full skill set. You’ll function as a single provider in the field—delivering both pre-scheduled visits and acute-response care in patients’ homes. This role combines the acuity of emergency and critical care nursing with the time to provide meaningful, patient-centered care where it matters most. And this is only the start. At VMSC, we’ll support your growth into new areas of practice, including assisting you in obtaining your Pre-Hospital Registered Nurse (PHRN) certification so you can expand into advanced EMS and critical care roles.

Requirements

  • Active Registered Nurse (RN) license in Pennsylvania (BSN preferred).
  • Minimum 2 years of clinical experience (emergency, critical care, acute care, or EMS preferred).
  • Strong clinical judgment, communication skills, and ability to work independently in dynamic field environments.
  • Passion for innovative, patient-centered care beyond the traditional hospital system.

Responsibilities

  • Function as the primary provider in the field, delivering both pre-scheduled visits and acute-response care directly in patients’ homes.
  • Perform advanced nursing assessments, interventions, and follow-up care under physician-directed protocols.
  • Provide real, meaningful care where it matters most — stabilizing patients, preventing unnecessary hospital readmissions, and supporting recovery.
  • Collaborate with physicians, advanced providers, and EMS professionals to ensure seamless care transitions.
  • Document care in electronic medical records and coordinate with the broader healthcare team.
  • Serve as a clinical leader in the community, bridging gaps between hospitals, EMS, and primary care providers.
