How you’ll make an impact in this role Coordinate holistic patient care by leading a multidisciplinary team to ensure every clinical specialist is aligned with the patient’s recovery goals. Implement and adapt personalized care plans, actively monitoring patient progress to ensure treatments remain effective and responsive to changing needs. Advocate for patient safety and comfort by executing clinical orders with precision and maintaining clear, real-time communication regarding patient status. Lead the discharge planning process in collaboration with the healthcare team to ensure a seamless and safe transition to home. What minimum requirements you’ll need Licensure / Certification / Registration: BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted. Licensed Registered Nurse credentialed from the Texas Board of Nursing required. Education: Diploma from an accredited school/college of nursing required OR a bachelors level degree not in nursing, an active RN License, and enrolled in an Alternate Entry Masters of Science in Nursing Program with a minimum of 1 year in the program completed. Requirements
