This RN position is essential in managing patient care through the pre-transplant stage and may also be involved in chronic disease management. Provide skilled observations, telephone triage, education, and health assessments of patients including history, appropriate interventions, and referrals. Foster continuity of care and facilitate transitions across outpatient and community providers on patient population as needed. Maintain knowledge of all applicable regulatory laws and requirements, including HIPAA, CMS Conditions of Participation, The Joint Commission, and UNOS to ensure regulatory compliance in nursing practice. Potential to support outreach efforts.

Requirements

  • Current licensure to practice nursing in the State of Michigan.Michigan
  • Three years of nursing experience as an RN within the last 5 years
  • At least 2 years experience caring for patients with chronic renal disease, general medicine (multisystem care), or relevant experience within the last 5 years
  • Transplant Certification (CCTC/CCTN) within three years of hire.
  • Critical thinking skills through the nursing process, excellent problem-solving skills, and demonstrated ability to set priorities.
  • Customer service, interpersonal communication skills, and the ability to build respectful relationships within and between units and individuals.
  • Adaptability to changing needs and priorities based on unit, department, and institutional goals/objectives.
  • Knowledge and skills providing patient and family education and reinforcement related to transplant processes, including but not limited to telephone intervention and face-to-face meetings in both individual and group settings.
  • Proficiency with typing and electronic health record documentation.
  • Ability to manage complex clinical issues utilizing assessment skills, protocols, and evidence-based interventions.
  • Willingness to adapt to different populations within Transplant Center after successful Orientation.

Nice To Haves

  • Experience in caring for kidney transplant patients within the ACU.
  • Experience in caring for Liver transplant patients within the ACU.
  • Experience with using the United Network Organ Sharing system.
  • 2-5 years' experience in the Ambulatory Care setting.
  • 1-3 years? ICU experience
  • Recent 3 years nursing experience in the inpatient setting
  • Knowledge of UMHS nursing policies and standards of practice.
  • Bachelor of Science Degree in Nursing.
