The Transplant Coordinator I enhances internal and external quality of services through quality assurance and improvement practices. This person has a holistic view of patient management. He/she develops and maintains nursing practice standards of care for a specific transplant population. Serves as a liaison and provides consultation and education services to support standards of care. Identifies and resolves clinical issues relating to the transplant patient. Collaborates with staff, the patient, and his/her family concerning patient evaluation and discharge planning. Participates in developing the interdisciplinary plan of care ensuring essential protocol medications, pertinent diagnostic tests, and frequency are ordered, completed and reviewed by physician. Maintains appropriate documentation for transplant candidacy and post-transplant follow-up care. Performs related responsibilities as required.
