This person has a holistic view of patient management. He/she develops and maintains nursing practice standards of care for a specific transplant population. Serves as a liaison and provides consultation and education services to support standards of care. Identifies and resolves clinical issues relating to the transplant patient. Collaborates with staff, the patient, and his/her family concerning patient evaluation and discharge planning. Participates in developing the interdisciplinary plan of care ensuring essential protocol medications, pertinent diagnostic tests, and frequency are ordered, completed and reviewed by physician. Maintains appropriate documentation for transplant candidacy and post-transplant follow-up care. Performs related responsibilities as required. Primary duties and responsibilities: Care Coordination and Patient Flow: 1. Coordinates communications with the outpatient transplant population such as transplant clinic visits, community referrals, telephone consultations, arranging for long-range beepers, outpatient treatments, and subsequent hospital admissions. 2. Develops and coordinates a system to organize the flow of patients from initial referral, through evaluation and education, to post-transplant follow-up and referral back to the community. 3. Collaborates with staff, the patient, and his/her family concerning patient evaluation and discharge planning. Clinical Standards and Quality: 1. Enhances internal and external quality of services through quality assurance and improvement practices. 2. Develops and maintains nursing practice standards of care for a specific transplant population. 3. Identifies and resolves clinical issues relating to the transplant patient. 4. Reports signs/symptoms of possible rejection immediately to physician and care team. Team Collaboration and Research: 1. Serves as a liaison and provides consultation and education services to support standards of care. 2. Participates in developing the interdisciplinary plan of care ensuring essential protocol medications, pertinent diagnostic tests, and frequency are ordered, completed and reviewed by physician. 3. Participates in daily patient rounds/huddles with transplant team. 4. Participates in collaborative, interdisciplinary research activities related to transplant patients. 5. Coordinates the collection of clinical data and patient samples for research studies. Basic Responsibilities of all EHC Employees: 1.
