About The Position

As a Regional Grant Coordinator, you will serve as the operational backbone of this initiative at the regional level, helping turn statewide strategy into on-the-ground action. You will manage grant implementation, support regional partners, and ensure alignment with state and federal requirements. In this role, you will act as the primary liaison between state agencies, regional coalitions, and statewide partners, guiding communities through planning, execution, and reporting efforts that expand access to care and improve health outcomes.

Requirements

  • Bachelor’s degree in public health, health administration, public policy, business administration, or related field Required
  • Experience in grant management, contract oversight, program administration, or related work Required
  • 5 years Experience working with healthcare organizations, rural communities, or public health systems Required
  • 3 years Strong understanding of compliance, budgeting, and reporting requirements for state or federal grants Required
  • 2 years Ability to manage multiple projects simultaneously with strong attention to detail Required
  • Excellent communication, facilitation, and relationship-building skills Required
  • Ability to work independently in a remote, field-based environment Required
  • Must reside within the assigned RHTP region and be willing to travel regularly within the region Required

  • Familiarity with Indiana’s rural health landscape, including hospitals, FQHCs, EMS, and community-based organizations Highly Desired

Responsibilities

  • Grant & Contract Management Serve as the primary regional contract manager for organizations receiving RHTP funding
  • Monitor compliance with state and federal requirements, including budgets, allowable expenses, and documentation
  • Ensure regional coalitions meet milestones, deliverables, and timelines
  • Identify risks or performance issues and escalate as needed
  • Maintain audit-ready documentation using state-approved tools and systems
  • Partner Coordination Collaborate with statewide Technical Assistance Providers (IHA, IPHCA, IRHA)
  • Participate in regional meetings, workgroups, and planning sessions
  • Align regional strategies with CMS and state priorities
  • Share insights and best practices across regions
  • Reporting & Performance Tracking Coordinate data collection and performance tracking aligned with CMS requirements
  • Ensure timely submission of regional reports to IDOH
  • Support partners in meeting reporting expectations and documentation standards
  • Monitor outcomes and support corrective action planning
  • Stakeholder Engagement Serve as the day-to-day liaison between IDOH and regional partners, including hospitals, clinics, EMS providers, and community organizations
  • Ensure regional efforts reflect local needs and priorities
  • Maintain strong communication with statewide leadership and teams
  • Program Implementation Help partners interpret program guidance and funding requirements
  • Track progress, identify barriers, and support problem-solving
  • Promote consistency through shared tools, templates, and frameworks
