About The Position

This is a FT on site position located at our Care Coordinator Center in Centerville Ohio.
The shift is 8am-6:30pm.
We provide superior care and superior services to patients at their end of life journey. Only those who have a heart for hospice will
succeed.

Requirements

• High school diploma or GED
• Two years or more experience in data entry
• Computer skills sufficient to properly document services and communicate effectively.
• Ability to drive during daytime, nighttime or inclement weather
• Valid Driver's License with Safe Driving Record
• State Minimum Automobile Insurance Coverage

Responsibilities

• Demonstrates excellent customer service skills to answer and address all phone calls coming into the referral and admissions department.
• Communicates accurately Ohio’s Hospice services to patients and families and referral sources including differences between curative treatment and comfort care.
• Schedules prospective patients to be assessed for admission to Ohio’s Hospice timely, coordinates the activities of the admission liaison effectively.
• Obtains accurate and thorough information to facilitate the intake process.
• Schedules admission appointments to admission liaisons coordinating workload and drive time effectively.
• Follows up on all pending referrals timely.
