About The Position

Serving as the trusted medical home for our patients, TCCH offers a wide range of services - from dental care, optometry, and pharmacy needs to behavioral health and endocrinology. Everything we do reflects our core values of Quality, Respect, and Service. OPPORTUNITY TCCH is growing and actively recruiting Referral Coordinator to join our Care Teams across all our care sites. Under the direct supervision of the Site Manager, the person in this position will serve as a primary point of contact for referral coordination with contracted managed health care plans. He or she assists members, clinic staff and providers with community resources referrals for patients. He or she responds to inquiries from members, clinic staff, plan, providers, hospitals, and external specialists regarding managed care. He or she educates clinic staff and others appropriate in managed care and provides support to the manager by meeting department and company goals and objectives. Work is primarily completed via telephone and direct contact with patients and staff.

Requirements

  • High school diploma/GED required.
  • Five years administrative experience working in a health care setting and two or more years of managed care experience preferred.
  • Bilingual (English/Spanish) language skills required.
  • Excellent written & oral communication skills.
  • Basic understanding of medical and insurance terminology.
  • Proficiency with computers and Microsoft products required.
  • Ability to complete assignments with minimal supervision.

Nice To Haves

  • Two or more years of managed care experience preferred.
