Job Summary: The overall intent of this position is to provide preferred access and system navigation support for Community Partner entities referring patients into care and improve two-way communication with referral sources. This is not a clinical position, but will improve access to clinical services for referred patients. Essential Job Functions: Preferred Point of Contact for community partners Regular contacts either via phone or in person with Community Partners with goal of strengthening relationships. Coordination and management of new referrals for community partners. Ensuring preferred assessment and entry into care and efficient coordination of services for these clients, including monitoring referrals and supporting client progression into appropriate treatment services. Work individually with referred clients to personalize engagement process By being friendly, welcoming, and exhibiting a positive attitude all times. Promote communication, connection, and linkage between referral sources, the appropriate treatment teams, and agency representatives By communicating with internal lines and community partners. Creating an environment for positive interactions between all stakeholders. Interface with a range of community professionals including social workers, health department, law enforcement, treatment providers, in order to ensure positive interactions between agency and community partners. Inform/educate Community Partners around on-going service continuum Work with Community Partners to identify, and vet, new service opportunities. Experience and Education Qualifications: Bachelor’s degree in social work, mental health, public health or related field preferred Basic Computer Skills (Microsoft Office) Organizational, excellent oral and communication skills Strong organizational skills, compassionate as well as assertive, excellent oral and written communication skills, customer service oriented Employment Requirements: Successful completion of background check including criminal record, driving record, abuse/neglect and fingerprint check. Completion of New Hire Orientation at the beginning of employment. All training requirements including Relias at the beginning of employment and annually thereafter. Current driver’s license, acceptable driving record and current auto insurance. Obtain trainings to assist in professional development meeting DMH Standards. Extensive knowledge of the policies, procedures, and regulations of the program to which the employee is assigned. Must be at least twenty-one (21) years of age to operate a company-owned vehicle. Must be at least twenty-three (23) years of age to operate a company-owned fifteen (15) passenger van. Must have reliable means of transportation in order to transport clients in personal vehicle. Physical Requirements: ADA Consideration - Sedentary work: Exerting up to 10 pounds of force occasionally (exists up the 1/3 of the time) and/or a negligible amount of force frequently (exists 1/3 to 2/3 of the time) to lift, carry, push, or pull, or otherwise move objects, including the human body. Repetitive movements of hands, fingers, and arms for typing and/or writing during work shift. Sedentary work involves sitting most of the time but may involve walking or standing for brief periods of time. Jobs are sedentary if walking and standing are required only occasionally, and all other sedentary criteria are met.
