About The Position

Are you someone with lived experience who wants to help others in recovery? As a Recovery Engagement Specialist, we are looking for someone who can provide support services, provide information, education, and guidance for clients, and promote wellness, recovery, and skill building to sustain better health. This role provides prevention training for managing stress and problems, making healthy lifestyle choices, recognizing and responding to relapse, managing crisis, and achieving and sustaining recovery. The Recovery Engagement Specialist assists clients with personal recovery goals, helps them develop natural support systems, and coaches clients to advocate for themselves within their healthcare systems. They also implement evidence-informed wellness interventions to support overall client well-being.

Requirements

  • Effectively communicate via written, verbal, in person and virtual methods.
  • Knowledge of role and function of various community resources.
  • Knowledge of and ability to instruct assigned clients in basic independent living skills.
  • Skill in customer service/experience techniques.
  • Skill in problem-solving.
  • Candidate must be a minimum of 21 years of age per funding requirements.
  • Associates degree required.
  • Two years of relevant experience working in the human services field.; as well as experience participating in their own personal recovery process in order to care for self or family member.
  • SUPR funded positions specifically require 1 year of recovery to be qualified.

Nice To Haves

  • Bachelor’s degree in human services preferred.
  • Education/experience equivalency may be considered upon approval.
  • WRAP facilitator with in twelve (12) months of accepting position is preferred.

Responsibilities

  • Provides group or individual training/interventions focusing on improvement of the client’s functioning and health-related issues (e.g., symptom/medication management, life skills, healthy eating, weight management, smoking cessation, recognizing the dangers of alcohol and other drugs, community reintegration, etc.).
  • Assists clients with developing their Wellness Recovery Action Plan (WRAP).
  • Provides wellness consultation.
  • Works in consultation/collaboration with clients, family members and other providers to implement plan.
  • Empowers clients to self-manage chronic health conditions and solidify wellness goals.
  • Assists clients with finding solutions utilizing motivational interviewing and finding motivation to complete their individualized recovery plan to achieve wellness goals.
  • Assists clients in overcoming any barriers to goals, including access to care.
  • Assist with navigating health care systems.
  • Links clients to community-based social support/services and develop collaborative relationships with other departments, services, and community health care agencies to facilitate and support wellness and quality care.
  • Provides ongoing case management services for assigned service recipients, including but not limited to assessment, planning, linking, monitoring, referral and advocacy.
  • Provides transportation assistance to help clients achieve and maintain wellness goals and initiatives.
  • Conducts intake for recipient entering services, including explaining services, gathering information needed on intake materials, ensuring service recipient understands all forms, assuring signature on appropriate contracts and consents, and submitting materials for processing, as assigned.
  • Conducts assessment and write assessment summaries, as assigned.
  • Assists service recipient in making discharge plans and provide follow up services as assigned.
