About The Position

Join our compassionate, patient‑centered care team as a Certified Recovery Specialist! We’re looking for a dedicated professional to support individuals receiving medication for substance use disorders within our integrated primary care setting. In this role, you’ll build trusting relationships, offer guidance based on lived‑experience, and help patients navigate their recovery journey while working side‑by‑side with medical providers and behavioral health consultants. If you’re passionate about reducing stigma, empowering patients, and strengthening recovery‑focused care, we’d love to meet you ESSENTIAL DUTIES AND RESPONSIBILITIES Perform work associated with recovery support services, therapeutic care, case management, treatment and rehabilitation of persons with drug and/or alcohol use through care coordination. Services and support will be delivered to substance use patients in the context of Medication Assisted Treatment (MAT). In accordance with Patient Centered Medical Home Philosophy, collaborates with team members including: patient; primary care provider; Behavioral Health provider and others to deliver comprehensive care to patients of Family First Health through: Patient Engagement and Empowerment Participates in case coordination by utilizing case management tools, such as strength-based case management, motivational interviewing and active listening to facilitate patient retention in care and move patients toward independence and increased autonomy. Assists patients in the management of their recovery by ensuring that the resources to address the patients’ needs are in place. Provides drug and alcohol education, risk reduction support, and medication adherence guidance. Promotes patient retention, self-sufficiency in treatment and utilization of available support through educating patients about service availability. Engages in brief relationship building and active listening with patients receiving treatment Provides supportive (peer-based) coaching and encouragement to patients contemplating or actively seeking help with substance use treatment and recovery. Assist in the development of comprehensive, individualized recovery plans incorporating resources that support patient goals and interests Will serve on-call duties based on on-call policy and departmental procedures. Provide hope and encouragement needed for the client in reaching his/her goal(s) for recovery Connection with other providers Identifies, develops and maintains positive linkages with agencies and social service organizations/services in the community and within FFH. Serves as a liaison between FFH interdisciplinary team, peers in other departments, managers, executives, patients and families, and other community agencies to ensure ongoing care for patients, to maintain connectedness, and for positive internal/external customer service. Facilitates warm handoffs between all available behavioral health and physical/dental health treatments. Creates and manages long-term individualized recovery plan by assessing patient’s target needs and reassess on timely basis, including making appropriate referrals for treatment and rehabilitation of drug and alcohol dependent patients. Attend mandatory treatment team/clinical round meetings to discuss the patient’s recovery process while in treatment including stabilization, behavior modification, MAT options, discharge and aftercare planning. Documentation Maintains timely and accurate documentation and tracking of all service provision, evaluation, and statistical data for the program; including detailed patient demographics, medical information, risk factors, and other information needed related to grant funding. Understands and adheres to all state and federal drug & alcohol confidentiality/HIPAA laws and related statutes. Attends and actively participates in both internal and external meetings as required. Attend trainings and conferences as determined by supervisor. Performs other duties as assigned and related to the position.

Requirements

  • Must have a minimum of two (2) years uninterrupted recovery
  • Must have or be eligible to pursue Certified Recovery Specialist certification (CRS)
  • Must be actively engaged in activities that support a recovery oriented lifestyle
  • High School Diploma or GED required.
  • Excellent customer service skills with the ability to effectively establish appropriate boundaries
  • Strong written and verbal communication skills with the ability to support individuals of all backgrounds
  • Ability to display critical thinking and exercise individual judgement and discretion in matters of significance
  • Ability to establish and maintain effective working relationships with patients, employees, and the public
  • Ability to be organized and work efficiently with strong attention to detail within a demanding work environment while prioritizing tasks
  • Thorough knowledge of the Guiding Principles of Recovery.
  • Knowledge of MAT and ability to perform duties in the context of MAT.
  • Knowledge of computer software applications (Microsoft Office Applications, others)
  • Knowledge of grammar, spelling, and punctuation as it relates to correspondence necessary to perform essential job functions
  • Availability to work a 40-hour week between the hours of 7:00 am and 7:00 pm.
  • Ability to set positive examples for team members through exceptional work performance including adherence to company policies and processes.

  • Ability to work in, and respond appropriately to a demanding environment including crisis situations
