About The Position

The Rapid Rehousing Specialist will provide comprehensive case management services to individuals and families experiencing homelessness, guiding them through the process of securing permanent housing as quickly as possible. The role involves working closely with participants to identify housing needs, assess eligibility for rental assistance programs, and develop individualized plans that ensure successful housing placement and long-term stability. The Rapid Rehousing Specialist will also connect participants to community resources and services, address barriers to housing, and provide ongoing support throughout the rehousing process. This position will be working with individuals 18-24 years old.

Requirements

Knowledge of the housing market, housing assistance programs (e.g., Section 8, rental assistance), and government benefits programs.
Understanding of homelessness, its causes, and the challenges faced by individuals and families in crisis.
Ability to develop and implement individualized housing plans and solutions to address participant needs.
Strong problem-solving skills to address barriers to housing and resolve conflicts.
Excellent interpersonal and communication skills to work effectively with participants, landlords, community partners, and colleagues.
Empathy and sensitivity to working with vulnerable populations, including those facing homelessness, mental health issues, substance use, and financial instability.
Strong organizational skills and the ability to manage a caseload, prioritize tasks, and meet deadlines.
Proficient in case management software, Microsoft Office Suite, and other tools for tracking case progress and documentation.
Ability to maintain confidentiality and adhere to ethical guidelines in working with sensitive participant information.
A valid driver’s license and reliable transportation (if required for the position).
Education: A Bachelor’s degree in Social Work, Psychology, Sociology, or a related field is preferred. An Associate’s degree or relevant certifications may be considered with equivalent work experience.
Experience: Minimum of two years of experience in case management, social services, or working with individuals and families experiencing homelessness.

Nice To Haves

Experience in housing services, eviction prevention, and working with rental assistance programs is highly preferred.
Experience in providing support to vulnerable populations, such as individuals experiencing homelessness, mental illness, or substance abuse issues.
Lived experience of homelessness, housing instability, or related systemic barriers will be recognized as valuable and relevant experience.

Responsibilities

Participant Assessment and Housing Placement: Conduct comprehensive assessments to evaluate the housing needs, barriers, and eligibility of participants for the Rapid Rehousing program.
Work closely with participants to identify available housing options, help them complete housing applications, and facilitate the process of securing permanent housing.
Assess participant strengths, challenges, and goals to tailor housing plans and identify necessary services for success.
Develop and Implement Housing Plans: Create personalized housing plans in collaboration with participants, outlining the steps and goals required to secure stable housing.
Develop specific timelines and milestones for participants to achieve, ensuring they understand their responsibilities and available support.
Monitor participants’ progress towards housing stability and make adjustments to plans as needed to ensure long-term success.
Provide Financial Assistance and Support: Administer financial assistance for housing-related expenses (e.g., rent, utilities, deposits) in accordance with program guidelines.
Ensure participants are aware of and utilize available financial support to overcome financial barriers to housing stability.
Work closely with landlords and participants to negotiate rent agreements, payment plans, and other housing-related needs.
Connection to Services and Resources: Connect participants to community-based resources, including mental health services, addiction treatment, employment support, life skills training, and financial literacy programs.
Assist participants in navigating service systems and accessing resources that will help them maintain housing and improve their quality of life.
Housing Stability Support and Follow-up: Provide ongoing support to participants once they are housed, including regular check-ins, crisis intervention, and referrals for additional services.
Monitor participants’ housing situations and help resolve issues that may threaten housing stability, such as landlord disputes, rent arrears, or other barriers.
Offer assistance with budgeting, financial planning, and household management to help participants maintain their housing in the long term.
Collaboration and Coordination: Work closely with other housing service providers, government agencies, landlords, and community organizations to ensure participants have access to necessary resources and services.
Facilitate communication and coordinate services to address participants’ needs holistically.
Case Documentation and Reporting: Maintain accurate and up-to-date participant case files, documenting assessments, housing plans, progress, and outcomes.
Track participant outcomes, including housing retention and improvements in participant well-being.
Prepare and submit required reports, including progress reports, participant outcomes, and any other program data, to ensure compliance with funding requirements and organizational policies.
Advocacy and Support: Advocate for participants in their dealings with landlords, housing authorities, and other agencies, ensuring that their rights are protected and that they receive the support they need.
Work to resolve conflicts that arise in the housing process and provide mediation where necessary.
Operate through the lens of trauma-informed care, recognizing the impact of adverse experiences and tailoring all advocacy, interactions, and support strategies to foster safety, build trust, and empower participants.
Crisis Management and Problem Solving: Provide crisis intervention when participants face immediate threats to housing stability (e.g., eviction, financial hardship, safety concerns).
Develop creative solutions to help participants overcome housing crises and find emergency housing when needed.
Assist participants in resolving issues that may affect their ability to maintain housing, such as eviction notices, financial difficulties, or family challenges.
Program Development and Community Outreach: Assist in the development and improvement of program policies and procedures to better meet participant needs.
Engage in outreach efforts to raise awareness of Rapid Rehousing services and assist with community education regarding housing issues and available resources.
