About The Position

1. Works with the Managers Quality to set priorities for MPP provider performance improvement for assigned specialties and regions. 2. Assists practices with quality improvement initiatives such as Patient Centered Medical Home (PCMH), Healthcare Effectiveness Data and Information Set (HEDIS) Clinical Quality and specific payor programing through evaluation of current performance and identifying areas for improvement. 3. Works with the Manager of Quality in developing and managing implementation plans to assist in practice transformation. 4. Responsible for educating and implementing process improvement tools (e.g. LEAN) in the physician practices for both current and new processes. 5. Conducts needs assessment at physician practices for readiness to implement technology (e-prescribing, Population Health software, Coding support tools, EMR, etc.) and to understand clinical workflows. 6. Following quality initiatives of contracted health plans, payers and governmental agencies, works with MPP Quality Team and practice to identify and achieve physician performance improvement outcomes.

Requirements

  • High School Diploma or equivalent
  • 3 years of hands-on outpatient office or ambulatory care experience.
  • Valid state driver’s license, reliable transportation and automobile insurance required and maintained throughout employment.

Nice To Haves

  • Associate Degree in health care or a current health care program certification.
  • 2 years experience as a Patient Centered Medical Home Champion in a practice or plan setting
  • 2 years supervisory experience in a physician practice
