About The Position

Special Citizens is seeking a detail-oriented Quality Improvement(QI) Specialist/Investigator to support the agency’s commitment to compliance and service excellence. Working closely with the Director of Compliance and Quality Improvement, this role is responsible for coordinating, conducting, and following up on incident investigations; preparing documentation for Incident Review Committee meetings; and documenting findings and reports for internal and external review. The QI Specialist/Investigator also supports internal auditing and quality reviews of the agency’s case management and programmatic operations to ensure compliance with all State, Federal, oversight, and agency regulations.

Requirements

  • GED/High School Diploma, bachelor’s degree in human services or related field preferred
  • Knowledge of OPWDD and Justice Center regulations
  • Progressive experience working with I/DD population
  • Demonstrated competency in written, verbal, organizational, computer, and computational skills, with knowledge of Microsoft Word, Outlook, Excel and EHR software
  • Must be detail oriented, self-driven and work both independently and as part of a team
  • Flexibility of work location with the ability to travel
  • Must have flexible working hours and be able to work outside normal business hours as needed
  • Maintains professionalism and confidentiality at all times
  • AMAP, SCIP, CPR and First Aid certification and maintenance of same, as required
  • Must have Incident Investigator certification or be able to complete course and pass certification exam within 4 months of hire.
  • Successful completion of criminal background history and fingerprint examination; a valid NYS driver’s license to Agency standard

Nice To Haves

  • Experience working with OPWDDs incident management software (IRMA) preferred

Responsibilities

  • Conduct, coordinate, and document incident investigations, as required
  • Review incident documents, communicating with and making recommendations to Directors, Supervisors, other Investigators, and agency Administration as needed
  • Compile, organize, and distribute incident packets and reports timely
  • Liaisons with all departments within the agency
  • Participates in internal audits of each agency program, including visits, writing deficiency notes, conferencing with managers to review deficiencies, implementing corrective actions and providing support for needed follow up
  • Alerts program management, supervisors, and the Director of CQI of risk areas involving service documentation
  • Safeguards the security and confidentiality of all documents and protected health information held by the Compliance Department in accordance with OPWDD and OMIG regulations.
  • Conducts staff orientation trainings, and training in any other areas of Compliance for agency employees and managers, as needed, in a group or a 1:1 setting as assigned
  • Ensure treatment plans are in accordance with OPWDD regulations
  • Active participation within Committees assigned by the Compliance Officer
  • Comply with applicable federal and state legal requirements, standards, guidelines, all Special Citizens policies and procedures including the Compliance Plan, Code of Business Conduct, HIPPA, and program and billing documentation standards
  • Completion of other tasks as requested
