About The Position

The Quality Improvement Coordinator is responsible for Center-wide quality improvement functions in compliance with internal and external standards and requirements, implementation of quality improvement standards and procedures, planning and providing Center-wide training on quality improvement topics, and coordination of assigned clinical programs as it pertains to the Quality Improvement Program.

Requirements

  • Bachelor’s degree required. Background in behavioral health, social services or not-for-profit field preferred

Ability to effectively present information to top management, public groups, and/or Board of Directors.
  • Reasoning Abilities: Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to interpret an extensive variety of technical instructions in mathematical or diagram form or deal with several abstract and concrete variables.
  • Mathematical Skills: Ability to work with mathematical concepts such as probability and statistical inference, and fundamentals of plane and solid, geometry and trigonometry. Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations.
  • While performing the duties of this job, the employee is regularly required to sit. The employee is occasionally required to stand; walk; reach with hands and arms; stoop, kneel, crouch, or crawl; and talk and hear.

  • Ability to interpret and implement quality improvement, JCAHO, CARF, DMHA standards;
  • Ability to create and implement training/in-services for Northeastern Center staff;
  • Ability to complete policies and procedures in a timely manner;
  • Ability to develop grants and complete RFP's at acceptance level;
  • Ability to effectively communicate QI standards and requirements to staff.

Responsibilities

  • Organize, coordinate, integrate and report on all assigned quality improvement and related activities for the Center.
  • Coordinate the review and implementation of quality improvement plans and actions approved by the Chief Executive Officer, Chief Operations Officer, the Executive Committee and the Professional Staff Organization.
  • Prepare quarterly and annual Quality Improvement reports.
  • Attend Board Planning Committee meetings to respond to questions.
  • Interpret CARF and DMHA standards relating to quality improvement to Administration, medical staff, program managers, coordinators, and PSO staff; advises of any changes in standards and provides training as needed.
  • Formulate and/or review and monitor quality improvement and/or related policies and procedures, and coordinate training in these areas for all Northeastern Center services and programs.
  • Attend quarterly meetings of the Indiana Council’s quality improvement subcommittee as appropriate and when relevant to ongoing Center needs.
  • Keep the Medical Director, Chief Executive Officer, and Chief Operations Officer advised of performance issues and problems relative to quality improvement within Northeastern Center programs, as requested, beyond the standard reporting system.
  • Assist in policy development related to issues of Quality Improvement.
  • Directly assure following documents/activities are completed and updated as appropriate: .1 Written Accessibility Plan .2 Satisfaction Surveys .3 Communication of Rights Policy .4 Consumer Grievance Policy .5 Consumer Funds Policy .6 Consumer Handbook .7 Triennial Survey
  • Oversee development, implementation, and review of policy and procedure relating to quality improvement associated with networking related activities for the Center.
  • Perform audit related activities on network agencies to determine conformance with quality standards as required per contract and Center policy on a biannual basis.
  • Provide ongoing reports to CEO and COO detailing results of such audits.
  • Communication with DMHA on audits conducted by other agencies along with findings.
  • Act as liaison with CARF
  • Oversee preparation of CARF survey and identifies areas needing improvement .1 Review and note updates to CARF standards manual .2 Track requirements relating to policy/procedure/manuals and monitors corporate compliance .3 Annually complete survey preparatory questions and evaluate responses .4 Coordinate compliance prior to actual survey, evaluate compliance and establish corrective action plan as needed
  • Coordinate improvement efforts and act as internal resource for staff in completing CARF related assignments.
  • Oversee completion of any QI reporting to CARF following survey
  • Ensure data is reliable and reproducible.
  • Ensure data is accurate and recorded properly.
  • Ensure data is as complete as possible.
  • Ensure data indicators and elements reflect what is intended to be measured.
  • Completes work related trainings timely as assigned or requested. (i.e. Relias, Risk Management, PSO, DCS, etc.)
  • Other duties as assigned
