About The Position

At Family Centers, our goal is to provide high-quality, accessible, and compassionate healthcare to the communities we serve. The incumbent contributes to ongoing quality improvement efforts by using data to inform patient care, operational processes, and regulatory compliance activities. This position supports Quality Assurance (QA) and Quality Improvement (QI) initiatives across the organization and helps foster a culture of continuous improvement.

Requirements

  • Bachelor’s degree in Healthcare Administration, Business, Analytics, Quality Management, or related field.
  • Strong analytical and problem-solving skills, including advanced proficiency in Microsoft Excel and data reporting tools.
  • Demonstrated organizational, project management, and time management skills.
  • Advanced proficiency in Microsoft Office 365 (particularly Excel and PowerPoint), Google Workspace tools, and Canva for professional data visualization and reporting.
  • Ability to translate complex data into clear, actionable insights for diverse audiences.
  • Ability to maintain strict confidentiality and ensure data integrity.
  • Strong interpersonal skills with the ability to collaborate effectively across multidisciplinary teams.
  • Ability to manage multiple initiatives simultaneously and meet deadlines.
  • Willingness and access to a reliable car to travel between sites and attend statewide meetings.

Nice To Haves

  • Experience in a community health center or FQHC environment.
  • Familiarity with Patient-Centered Medical Home (PCMH), Joint Commission Standards, and HRSA quality performance measures.
  • Experience with Epic electronic health record system.
  • Experience preparing UDS or other regulatory reports.

Responsibilities

  • Maintain, organize, and ensure the integrity of centralized clinical and operational data libraries.
  • Collect, validate, and analyze healthcare data to support QA and QI initiatives aligned with HRSA and organizational performance goals.
  • Prepare and present monthly clinical performance reports, executive summaries, and dashboards that clearly communicate trends, outcomes, and opportunities.
  • Lead and manage Quality Improvement initiatives, including project planning, milestone tracking, outcome measurement, and progress reporting.
  • Prepare and organize data for annual UDS (Uniform Data System) reporting and other regulatory submissions.
  • Identify opportunities for process and performance improvement through data analysis and benchmarking.
  • Collaborate with clinical and operational teams to promote transparency, accountability, and shared quality goals.
  • Participate in statewide meetings and contribute to strategic performance improvement discussions.
  • Travel between FCHC sites as needed.
  • Other duties as assigned.
