Maximize resources to enhance Lifepath Systems operations and support client’s strengths and needs, and helping to navigate medical, self-help, social, vocational, financial, educational, and other individualized services. Demonstrates integrity in role and by ensuring state standards and Lifepath Systems policies and procedures are met (i.e., timeliness, documentation, case management). Work independently with integrity and collaborative as a team member and community builder. Continuously improve in measurable ways on how to support individuals in service, community, and LifePath Systems services as a leader in the QMHP-CS role.
