Performs supported housing, routine case management, skills training, medication training and support, and psychosocial rehabilitation services based in trauma informed and person driven care model. Provides person directed case management to assist in navigating access to safe affordable short term and/or permanent housing. Provides person directed skills training and psychosocial rehab to develop skills needed in order obtain and maintain stable and safe housing and to feel successful in their home and community. Requires use of initiative and independent judgment to provide interventions in the community, home, and school to individuals on caseload. Is responsible for initiating and collaboratively completing uniform assessments then organizing and documenting the plan for services to meet the individual’s identified needs. Must be effective at troubleshooting and organizing support, interventions, and services to achieve outcomes. Must have the ability to work independently and collaboratively as needed and have effective communication with supervisory staff when necessary. Expectations include timely and thorough documentation, time management and organizational skills, effective scheduling, ability to efficiently manage clinic flow, adhere to crisis procedures and be available to assist with crisis and other coverage areas as needed. Must understand and implement to fidelity SAMSHA Supported Housing program as well as our statement of work regarding rental assistance. Must work well with community partners such as landlords, leasing agents, the local homeless coalition, etc. Must understand and follow all rules for assisting with Section 811 applications as well as other public housing opportunities.
