The Continuity of Care (COC) Qualified Mental Health Professional (QMHP) Community Services (CS) reports directly to the Access and Diversion Supervisor. This position is responsible for facilitating the continuity of care for individuals served by LifePath System's behavioral health department, including coordination between state and local psychiatric facilities. Tasks include providing requested records to appropriate facilities, discharge planning and coordination, completing intake appointments for individuals upon discharge from local hospitals coming into LPS outpatient services to include biopsychosocial, financial assessment, needs assessment, recovery plan, TRR and facilitation of initial diagnosis. This position is responsible for facilitation of referrals and linkage to additional services as needed, collecting, update of assessments or documentation needs for established individuals, and recording data needed to evaluate performance measures. This position works closely with hospital staff and other stakeholders in the community and is expected to maintain professional relationships with all collateral contacts. The goal of this position is to link Collin County residents with appropriate Behavioral Health Services. Must be able to take initiative in completing complex tasks and projects. Works under moderate supervision with limited latitude for the use of initiative and independent judgment. Other duties as assigned to meet program and individual needs. This position exemplifies service excellence by providing an overview of LifePath Systems services and then connects to outpatient behavioral health services. Services will be provided with a trauma-informed and person-centered approach. This position practices good stewardship of resources by ensuring all required training is completed prior to due date and center-issued equipment is maintained in good condition. The COC QMHP-CS will link individuals to additional resources as needed to support their goals. Integrity is necessary to ensure adherence to the Health and Human Services Commission, Texas Administrative Code, contracts, applicable grants, and Lifepath Systems policies and procedures. This position will work closely with fidelity managers and leadership to ensure that all gaps are reconciled. This position builds and maintains positive relationships with hospital staff, mental health professionals, and other community stakeholders. Must demonstrate professional conduct with Peers, Leadership and Support teams at all times. In addition, close working relationships and professional conduct while coordinating care with outside entities is required. Working independently and collaboratively to maintain appropriate boundaries and foster better outcomes for individuals served and facilitating improvement in measurable ways, is central to LifePath Systems mission to serve. This position must have the ability to assess and de-escalate behavioral health crisis situations in a calm and respectful manner. May be listed as the primary contact as COC liaison with HHSC to support COC activities and delegate COC responsibilities. This includes coordination and participation in coordinating discharge planning with community partners involved in the individual’s care and development of an outpatient management plan for individuals on 46C commitment. The COC liaison is the responsible workforce member for individuals discharged/furloughed until a reassignment of these duties are documented in the individuals’ record. Specific Targets: Engagement with individuals served and exceptional customer service will be provided and documented in the electronic health record a minimum of half of time worked. At least 90% of service notes are signed within 2 business days of service. Full chart reviews receive a score of 80% or higher following the Golden Thread. Provides person directed services to assist in navigating access to medical, social, vocational, financial, educational, and other individualized services with a focus on access to care and diversion from incarceration. Must understand criteria for LifePath Systems services and be able to explain to individuals served as well as community partners. Attends monthly team meeting.

  • Bachelor’s degree in psychology, social work, nursing or related field from a four-year college or university that meets the Texas Administrative Code for requirements for a Qualified Mental Health Professional- Clinical Services (QMHP-CS).
  • Must have a valid Texas driver’s license.
  • Experience in service coordination including community resources.
  • Ability to foster collaborative relationships with key interfacing teams, be a self-starter who works closely with all team members and takes initiative in completing tasks.
  • Ability to ensure effective communication with the multi-disciplinary team and across a variety of community resources.
  • Will work under general supervision with moderate latitude for the use of initiative and independent judgement as well as identifying when a licensed professional need to be consulted.
  • Knowledge of the social services system with the ability to connect individuals with appropriate services.
  • Must be organized, detail oriented, computer proficient, and able to write clearly and professionally.
  • Ability to respond with individuals and staff with patience, empathy, and understanding.
  • May have a Master’s degree in counseling, social work, nursing or related fields.
  • At least two years of clinical experience in the treatment of individuals with mental illness and/or substance abuse disorders preferred.
