About The Position

JOB SUMMARY The Registered Nurse – Quality Assurance is responsible for analyzing data integrity and consistency with OASIS documentation and assessment processes. This position will work with clinical staff to clarify documentation and data integrity issues. This role is responsible for the clinical workflow in HomeCare HomeBase and maintaining tasks as they are assigned. DUTIES & RESPONSIBILITIES Patient Care Prospectively reviews all OASIS assessments to ensure appropriateness, completeness, and compliance with federal and state regulations and organizational policy. Additionally, reviews the plan of care, face-to-face-review and approval. Provides feedback and education to field staff on documentation and regulatory requirements and assists with OASIS education and training for staff. May develop education or training for identified trended opportunities to strengthen documentation. Ensure appropriate ICD-9 and ICD-10 coding and sequencing as it relates to the patient’s medical condition including any co-morbidities. Reviews visit utilization for appropriateness of care guidelines and patient condition. Reports potential financial losses and/or underutilization to the clinical manager or designee. Will work with the Billing Department to resolve claim issues that relate to OASIS or plan of care. Participates in quality improvement, corporate compliance activities, orientation for new staff OASIS training, documentation and care coordination across disciplines. Prioritizes workflow and tasks, audits chart activities, and effectively communicates findings and trends with staff and leadership for the purpose of increasing awareness of standards of care, identifying deficiencies and reinforcing compliance. Additional Duties Participating in the HHA's quality assessment and performance improvement program and HHA-sponsored in-service training. 2. May be required to do field visits with clinical staff for the purpose of OASIS, care plan creation, and comprehensive assessment and training. Ensures that arrangements for equipment and other necessary items and services are available. Instructs, supervises and evaluates home health aide care provided every 14 days. Supervises Licensed Practical Nurses (LPNs)/ Licensed Vocational Nurses (LVNs) and/or Home Health Aids (HHAs)/Certified Nursing Assistants (CNAs), as directed. Requirements

• Must have graduated from an approved school of nursing and be licensed in the state where practicing.
• Minimum of two years of experience, at least one of which is in the area of public health or home care nursing is preferred.
• Maintains a current CPR certification.
• The ability to drive and/or have access to transportation for agency related travel (e.g. community partners, patients, etc.) is an essential function of the job.
• Must have and maintain a valid driver’s license, maintain automobile insurance coverage and have access to an automobile.
• Self-directed and able to work with minimal supervision.
• Demonstrates excellent observation, problem solving, verbal and written communication skills; nursing skills per competency checklist.
• Shows ability to organize and prioritize workload independently.
• Current OASIS certification or the ability to obtain within six (6) months of hire.
• Prolonged or considerable sitting, occasional walking or standing.
• Able to lift, position, or transfer patients.
• Able to lift supplies and equipment.
• Considerable reaching, stooping, bending, kneeling, or crouching.
• Visual acuity and hearing to perform required nursing skills.
