About The Position

No Nights, Weekends or Holidays! UI Heart and Vascular Center is seeking a 100% Nurse Clinician to support our Pulmonary Hypertension Program. The Nurse Clinician within the Advanced Heart Failure Program plays a pivotal role in providing comprehensive care and support to patients facing complex cardiac conditions. This role combines clinical expertise, patient education, and coordination of care to optimize patient outcomes and enhance their quality of life. The Nurse Clinician collaborates closely with multidisciplinary teams, including physicians, nurse practitioners, social workers, and other healthcare professionals, to develop and implement individualized care plans tailored to each patient's unique needs. Responsibilities include conducting thorough patient assessments, managing symptoms, administering treatments, and offering guidance on medication management and lifestyle modifications. Additionally, the Nurse Clinician serves as a primary point of contact for patients and their families, offering emotional support, education, and resources throughout the continuum of care. Requirements

Baccalaureate Degree in Nursing is required.
Minimum 6 months providing direct nursing care or management/supervision of cardiovascular medical/surgical patients OR Minimum 1 year providing direct nursing care/ care coordination within an outpatient setting required.
Excellent written, verbal and interpersonal communication skills.
Experience and skills with electronic health record is required.
Current certification in BLS required by start date.
Current license to practice nursing in the State of Iowa required by start date.

Nice To Haves

3 years providing direct nursing care or management/supervision of cardiovascular medical/surgical patient population.
Experience managing the care coordination of patients in an outpatient setting and/or within cardiothoracic surgery desired.
Experience in an academic medical setting desired.
Previous experience using EPIC desired.

Responsibilities

Collaborate with nursing, medical staff and other health professionals in the development and review of standards and in planning, implementing, and evaluating patient care for the patient population with regard to the condition and age of the patient.
Provide direct nursing care when necessary.
Develop and implement individualized care plans in collaboration with the multidisciplinary team, tailored to each patient’s specific needs and goals.
Monitor and manage symptoms of advanced heart failure, including dyspnea, edema, fatigue, and chest pain.
Administer medications, including intravenous and subcutaneous therapies, and provide patient education on medication management, potential side effects, and adherence.
Oversee patient flow through the UI Health Care continuum of care on both the inpatient and outpatient setting.
Facilitates care of patient(s) requiring admission, coordinating necessary admission procedures, pre and post procedure care.
Coordinate and facilitate patient education sessions on pulmonary hypertension management, self-care strategies, and lifestyle modifications to promote optimal health outcomes.
Facilitating learning and provides consultation to those in external agencies, communities and to other health care professionals.
Supporting implementation and evaluation of staff education and orientation programs, coordinating and presenting.
Facilitating the educational experience for health care students and monitors the care delivered.
Assist in the coordination of diagnostic tests and procedures, such as echocardiograms, cardiac catheterizations, and cardiac imaging studies.
Collaborate with social workers, case manager, and community resources to address psychosocial needs, financial concerns, and access to support services for patients and their families.
Independently coordinate and facilitate patient care between multiple disciplines within both the confines of UI Health Care and the community (including home health agencies, referring physicians and other community support services).
Facilitate continuity of care.
Perform telephone triage of patient calls and facilitates medication refills as needed.
Provide cross-coverage to other HVC Nurse Clinicians PRN.
Provide ongoing support and counseling to patients and families, addressing emotional and psychological concerns related to living with pulmonary hypertension and advanced heart failure.
Participate in interdisciplinary rounds, case conferences, and team meetings to discuss patient progress, treatment plans, and care coordination efforts.
Document patient assessments, interventions, and outcomes accurately and comprehensively in electronic medical records, ensuring compliance with regulatory standards and organizational policies.
Stay updated on advances in heart failure management, evidence-based practices, and clinical guidelines to enhance knowledge and skills in cardiac care delivery.
Contribute to quality improvement initiatives and research projects within the Advanced Heart Failure Program and the Heat and Vascular Center to enhance patient care delivery outcomes.
Serve as a resource and mentor to nursing staff, providing guidance and support in managing complex cardiac cases and fostering a culture of excellence in cardiac care delivery.
Provide input into financial management activities including operational budgetary decisions and cost containment.
Participate in the ongoing implementation of current regulatory standards required to meet state and national agency accreditation.
Assist with the designated quality database, including compiling and entering quality data.
Use relevant nursing research to improve clinical practice and investigate nursing care problems related to a specific patient population.
Interact with staff, patients, medical staff, peers, and others in a professional and cooperative manner.
