DHHS Nebraska is seeking a dedicated health educator to enhance access to diabetes self-management, education, and support programs and improve the quality of care among healthcare providers to improve outcomes for Nebraskans with diabetes. The coordinator will play a pivotal role in enhancing access to Diabetes Self-Management Education & Support (DSMES) programs, ensuring that more individuals with diabetes have the opportunity to engage with and benefit from these programs. They will also work closely with healthcare teams to improve their diabetes management practices, potentially through training, sharing best practices, and incorporating evidence-based strategies into care routines. The role involves overseeing the successful implementation and administration of federal grants, including developing grant applications, budgets, and work plans, and ensuring compliance with federal and state regulations. The coordinator will also foster collaborative relationships with internal and external partners, such as healthcare providers, local health departments, and community organizations, to identify community resources and gaps in diabetes care. Additionally, a key objective is providing technical assistance and training to various stakeholders, including contractors, healthcare clinics, and local health departments, to equip them with evidence-based strategies, policies, and practices. This role also oversees the development, implementation, and monitoring of contracts and subawards, ensuring adherence to regulations and support for broader program goals.
