About The Position

The Public Health Program Nurse (PHPN) will be instrumental in enhancing the health of communities across Western New York. This role involves providing essential training, technical assistance, resources, guidance, and clinical guidance on community health services to various partners, including local health departments and community-based organizations within the 17 counties of Western New York. Key responsibilities include monitoring public health data, identifying public health issues, contributing to health equity programming strategies, and evaluating implemented activities. The PHPN will also foster collaboration among Community Health programs and stakeholders to improve health outcomes for all age groups. Additionally, the role encompasses guiding health education and promotion activities, such as the implementation of the Prevention Agenda and the Chronic Disease Action Plan, and may require participation in public health emergency response activities.

Requirements

  • A Bachelor’s of Science degree in Nursing AND a license and current registration as a professional registered nurse in NYS AND four years of experience planning, evaluating, or administering health services or teaching in a health related program; OR an Associate’s degree in Nursing AND a license and current registration as a professional registered nurse in NYS AND six years of experience planning, evaluating, or administering health services or teaching in a health related program.
  • Valid and unrestricted authorization to work in the U.S.
  • Reside within a reasonable commuting distance of the official work location and must also be located in or willing to relocate to one of the following states: New York, New Jersey, Connecticut, Vermont, or Massachusetts, prior to hire.

Nice To Haves

  • Familiarity with family health and/or chronic disease activities including maternal, child, and reproductive public health services; and prevention and control activities related to cancer, diabetes, cardiovascular disease, and/or asthma.
  • Experience in oversight and evaluation of public health and preventive health programs, as well as promoting public health messages.
  • Experience supervising staff.
  • Experience managing multiple tasks, developing, and delivering educational materials, coordinating training activities, and contributing on policy development.
  • Excellent organizational, interpersonal, and written and oral communication skills.
  • Proficiency with computer-based software packages such as Microsoft Office, Outlook, etc.

Responsibilities

  • Provide training, technical assistance, resources, guidance, and clinical guidance on community health services to local health departments, community-based organizations, and other community partners within the 17 counties of Western New York.
  • Monitor public health data.
  • Contribute to the identification of public health issues.
  • Contribute to strategies to promote health equity programming.
  • Evaluate activities implemented.
  • Facilitate enhanced collaboration and coordination between Community Health programs, providers, and other key stakeholders to improve health outcomes for infants, children, adolescents, and adults.
  • Provide training, technical assistance, resources, guidance, and clinical guidance to local health departments, community-based organizations, and other community partners regarding health education and health promotion activities including implementation of the Prevention Agenda and implementation of the Chronic Disease Action Plan.
  • Participate in public health emergency response activities as needed.
